TEAM SPORTS
All students engaging in team sports must be up to on their physicals. Click the link below for the form. Form Expires in one year.
Sports Physical Requirements
Due to new State of Florida requirements, all students planning to participate on a school sports team must have a completed EL2 form on file.
Additionally:
High school students only are now required to submit an EKG along with their EL2.
